Prior to the issuance of the Certificate of Occupancy, the Zionsville Fire Department requires each contractor to provide the following documentation when applicable:
-The Record of Completion is required for all Fire Alarm Systems
-The Above-ground Contractor’s Material and Test Certificate is required for all Sprinkler Systems and Standpipe Systems
-The Under-ground Contractor’s Material and Test Certificate is required for all Private Fire Service Main installations
-Installation Certificate on all Special Hazard Fixed Fire Suppression Systems
-Type I & II Exhaust Hood Air Balance Test Report
-The Fire Pump Manufacturer’s Certified Pump Test Report
-Affidavit for 90 Minute Emergency Lighting Test
-Completed and approved Fire Safety and Evacuation Plan
-The Tank Tightness Report for Underground Storage Tanks
-As-Built CD or Emailed file of Civil Drawings & Architectural Drawings in AUTOCAD Format
-MSDS for all Flammable and Combustible Liquids
-Fire Resistive Rated Construction Affidavit
-Emergency Contact Information Sheet
